This website and print survey giveaway competition is being held and promoted by Adventist Health Care Limited ABN 76 096 452 925.
Competition Rules
Opening and Closing Dates
All entries will be collected from the GP Feedback survey participants that opt in to providing their details. Entries will be accepted between Monday 25th November 2024 to Friday 31st January 2025.
After the designated submission period has concluded three entrants who have participated in the survey will be selected at random and deemed the winners. The winners will be drawn at random from all survey participants that opt into the giveaway and provide contact details.
Who May Enter
The only people who may enter this competition are those that have completed the GP Feedback survey from Adventist Healthcare and have opted in to sharing their details.
How to Enter
An entrant who participates in the GP Feedback survey and provides the information requested between Monday 25th November 2024 to Tuesday 3rd December 2024 (EST) will be entered into the giveaway. The entrant must provide answers in the GP Feedback survey and submit it with their contact details. Entries can only be submitted via our online Survey Monkey portal. Only one entry per person is accepted. Each entrant will be required to provide contact details.
Conditions of Entry
An entry to the website survey giveaway will only be valid if the Entrant agrees to be bound by these terms and conditions.
Instructions on how to enter and other details contained within promotional advertisements for the feedback survey form part of the conditions of entry.
AHCL reserves the right to verify the validity of entries received, and to disqualify any entrant who submits an entry that is not in accordance with these terms and conditions. Entries may be disqualified without notice if they are perceived as inappropriate in any way.
Participants may only submit one survey giveaway entry, multiple entries will not be recorded and only the first entry recorded will be accepted.
Selection of the Winner and Prize Details
The winning entry will be drawn at random from all entrants who participate in the website survey giveaway, while abiding by the rules of the competition. The winner will be notified within one week of the close of the competition, via their contact details provided within the survey and will be given two weeks to contact AHCL to provide details of where to send their prize.
There will be three (3) winners. First prize will receive a $300 Coles/Myer giftcard, and two other entries will win a $50 Coles/Myer giftcard.
The prize must be accepted as offered and is not exchangeable, transferable or redeemable for cash.
If any aspect of the prize cannot be delivered for any reason, AHCL reserves the right to substitute the prize for any other item of equal or greater value.
If the prize remains unclaimed within two weeks of the competition closing, the Prize will be considered forfeited and may be used by AHCL for an alternative purpose, entirely at the discretion of AHCL.
Privacy
Personal information collected by AHCL in conjunction with this competition will only be used to identify the winner of the competition and for AHCL's future marketing and promotional activities.
All personal information collected by AHCL in conjunction with the prize draw will be handled and used in accordance with the AHCL Privacy Policy available at www.sah.org.au/website-privacy.
Limitation of Liability
AHCL, its employees, agents or contractors will not be liable for any loss or damage whatsoever which is suffered (including but not limited to indirect or consequential loss) or for personal injury which is suffered or sustained, as a result of this prize draw or the Prize, except for any liability which cannot be excluded by law. In the event that any such liability cannot be excluded by law, the total aggregate liability of AHCL will not exceed the total cost of the prize (being up to $300).
