Transfers to Another Facility

Your doctor may decide that you need to be transferred to another facility, such as a Rehabilitation Hospital for example, for continuation of your care. Our Discharge Coordinator or your Case Manager will ensure that all necessary arrangements are in place to ensure a smooth transition of your care. If necessary, transport via our Hospital Transport Vehicle or the NSW Ambulance Service will also be arranged for you.
