Outpatient Heart Health Program (Phase II)

 

ESSENTIAL CARE FOLLOWING A CARDIAC PROCEDURE OR EVENT

The program runs three days a week for six weeks, providing exercise and education sessions to increase your fitness, reduce cardiac risk factors and increase confidence in a safe monitored environment. Sessions are held on Monday, Wednesday and Friday in the morning, or Monday and Thursday evenings.

All sessions are supervised by an exercise physiologist and cardiac registered nurse.

Programs are not exclusive to previous Sydney Adventist Hospital patients and are covered by most health funds.

The program involves the use of stationary bikes, treadmills, arm ergometer and free weights. Each client has an individually structured program with assessment of blood pressure, ECG and heart rates, before, during and after exercise. 

Cardiac Rehabilitation staff will receive an initial referral from your cardiac case manager. On discharge please call as soon as possible to organise your booking. Once you have been booked in, the department will then obtain final consent from your cardiologist.

 

 

Maintenance Exercise Program (Phase III)


This is a continuation of the Phase II Program, and is an independent program for those wishing to maintain their fitness level under medical supervision in a caring and supportive environment.

 

 

Conditioning for Life


Conditioning for Life is an exercise program for those with reduced physical function, for example patients with:

  • Reduced heart function from any cause
  • Chronic lung disease/limitations
  • Reduced muscle strength/fitness due to chronic conditions or following an illness.

Sessions are individualised, and tailored to each patient. They involved muscle strength, co-ordination and balance with the desired outcome to improve physical function.

The program is held on Mondays and Fridays and runs for 7 weeks. All sessions are supervised by an exercise physiologist and/or cardiac registered nurse and are covered by most private health funds.

Cardiac Rehabilitation staff will obtain consent from your GP or cardiologist before you commence this program.

 

 

Healthy Weight Management Program


This program is run by both Cardiac Rehabilitation and San Nutrition. It is a 5-week program aimed at improving your nutrition and increasing your physical activity. It allows participants to learn, plan and progress towards a healthier lifestyle. The program includes a total of five sessions with either an exercise physiologist or a practicing dietitian. Please phone 02 9487 9470 for more information, and to make your initial appointment with the Exercise Physiologist, who will then send you a registration pack.

 

Cardiac Patient Education DVD

The San recognises that lifestyle choices play an important part in preventing heart disease or further cardiac problems. To make the adjustment to a healthier heart life a little easier, we have an educational DVD available for sale.
